When things do go wrong in an aesthetic treatment. Reflections after such an event and yes we do need to talk about it.

We love aesthetics and especially when things go right, and the patient is delighted with their results. But what about when things go wrong? I am a South African and have been an advanced nurse practitioner for 29 years. I worked in trauma, medicine, intensive care and GP practice. I have seen and lived through most things, especially in trauma and honed my skills in South Africa. One of the most violent countries with horrific atrocities.
Fast forward and I am standing in my own clinic doing aesthetics full time. I have found my niche between medicine and beauty. This place of magic where you can truly help people regenerate, restore and beautify them. Making a true difference to their lives in areas that really concern them. It is just such a humbling and satisfying experience especially when things go right. So, Tuesday is just another great day in clinic and looking forward to seeing my various clients. Mrs. J is booked in for lip fillers. We had quite a journey thus far since I started to treat Mrs. J several months ago. Her main complaint was looking and feeling tired all the time. She presented with dull bruised skin with deep dark circles under her eyes and lots of fat pad loss – a natural occurrence in ageing. It was affecting her confidence and she was uncomfortable in her own skin. During her first treatment I noted that she bled profusely after every injection and as if her skin tissue was disintegrating under my needle. I completed only part of the planned treatment and advised her to see her GP for routine bloods. She also bruised horrifically after my injections, and it took a long time to resolve which is equally unusual. Her GP advised her that her bloods are normal but from years working in practice I knew that this couldn’t be the case. I asked her to bring her results with her and noted that some of her levels were at the low end of normal, specifically her iron count and folate levels. So even if her bloods were at the very low end of normal it was clearly not normal for her as she had all the symptoms of abnormality. I prescribed her iron treatments for anaemia and also folic acid for her low folic levels for at least 3 months. Within a week she already felt significantly better and messaged me with her delight. She got used to feeling ill so long she thought it was normal. We left it a few months and she came back for her follow up treatment as planned. Result, no bruising and we completed her tear trough treatments without any incident. On Tuesday the woman that then walked through my door looked amazing. Her hair and skin were shiny, her eyes were laughing and she was feeling her old self. Even her family had noticed. So, we decided after the success of her tear troughs to tackle her lower face and I advised her to have lip fillers as she had down turned corners, lines through her lips and knew that it would significantly result in looking less aged. This then brings us to the date on Tuesday.
We are going to perform her lip fillers and both of us are very excited. I so love helping woman looking and feeling their best. I continued to inject Mrs. J, and all worked out nicely. The usual culprits of slight swelling and discomfort in the lips are present but is expected as we have caused trauma with multiple injections to a sensitive area. The lips are full of nerves as it is a very sensory organ by nature. It is a part of our body which we use for pleasure, sensation, eating and talking. The mouth has to perform a multitude of movements to form words so is always more sensitive when injected. When Mrs. J left she looked as expected but as per usual I give advice to ice them for a bit and to take pain killers if needed but also to ring if there are any concerns.
It is Saturday early evening (4 days later) and I get THE call; one I did not expect but I also don’t ever want to receive. It is Mrs. J and her top lip is very painful. She described it as if someone just punched her in the mouth and she expected it to be less painful by now as everything else has settled. She was icing it constantly because the pain would not go. Things are not normal and are starting to go wrong. My heart was in my throat and I asked Mrs. J to come back immediately and I rush to meet her. As I opened the door, I recognised the vascular occlusion, every practitioner’s worst nightmare. This is when the blood is no longer able to pass through a blood vessel. It can be caused either by an internal obstruction such as a blood vessel due to the filler or externally where the filler is compressing the blood flow. If left untreated the loss of blood supply causes the tissue to die in the surrounding area. So, this is as serious as it gets. The common symptoms are increased pain and change in skin colour. Unfortunately, the problem was further exacerbated by Mrs J applying ice throughout which further causes spasm to the tissues. However, that brought her relief and is not a fault on her part.
As I stood there and started to prepare Mrs. J for the emergency procedure millions of thoughts were swirling through my head. The most immediate thought is stay calm I have got this. I have been trained by the very best people in the country and Europe. I belong and have been trained by ACE (Aesthetic Complications Group) and have all the protocols. I belong to Save Face who have assessed and accredited my business and practice as safe. I had quality training by renowned injectors such as Dr. Tim Pearce, Dr. Sabrina Shah Desai and Dr. Ash Labib, Dr. Lee Walker and too many others to mention. Every training event and course always covers the risk areas and hone our skills to recognise such an event. I have done a Masters in aesthetic medicine and have been assessed by multitude of doctors and plastic surgeons and passed that with distinction. I have trained on cadaver courses in Spain and London and understand anatomy very well. However, I also know from these cadaver courses and studying anatomy handbooks that there are many exceptions to arteries and veins and that they are not anatomically the same for any patient. Besides this, I have worked in much worse areas and situations in my 29 years. I was a flight nurse to recover patients who stepped on landmines, handled multitude of stab wounds and inserted chest drains into patients with collapsed lungs. I have delivered breach babies in the community without doctors’ present and have run cardiac arrests on babies, children and adults. I worked in intensive care and we busted clots that cause cardiac arrests and strokes. I have so GOT this!! It was only seconds, but all these crowded into my head followed by the familiar click of calmness and experience in events like this. I was able to calmly explain to Mrs. J what was happening and what needs to be done. I have already drawn up the hyaluronidase whilst waiting for her imminent arrival. Hyaluronidase is used to dissolve filler and the procedure known as hyalase. I patch tested her as it was not a dire emergency. This is to make sure that she does not have an allergic reaction to the hyaluronidase which would be a bit of double trouble. Luckily, I am also prepared for that eventuality as I carry adrenaline, antihistamine and hydrocortisone if an allergic incident should occur.
After the patch test, which was thankfully negative, I explained the procedure and then proceeded to hyalase the affected area. Within seconds the colour improved and then resolved. The pain sensation subsided. All was well. I kept Mrs. J for another hour to make sure she is okay, that her symptoms have fully resolved and that she does not have an allergic reaction. I then booked her for a follow up appointment the next day (Sunday morning) and had a stiff drink and a long reflection.
I wanted to know what I could have done differently and learn from it. The first thing is that I wish that nobody ever has to go through such an event, and I am so sorry that it happened in the first place. I am comforted by the fact that it can happen to anybody and many experienced injectors. More and more people are having fillers done which leads to more of these events. I also know from the many cadaver courses and anatomical books that no two people have the same anatomy and why we have to take precautions. Despite this occurrence I am reassured that I have still delivered by best and could not have changed a thing. I inject small amounts of filler very slowly, whilst vigilantly watch for any changes in skin colour. I aspirate (draw back on the syringe to see if there are any blood in the syringe in case it is in a vein). This was negative throughout the procedure. I look for veins before I inject to avoid any events such as this. I massaged and looked and there were no areas of concern at that time. There was a small bruise to her lip but that can be expected and did not concern me. So, my learning on injection technique is that I could not improve on anything I already do. I follow all the principles of a safe injector. I safeguarded which is why she was able to contact me. I am incredibly thankful for my ongoing training with exceptional people in the field and do not shy away from costs and do not cut any corners.
One thing I wish could have been different for Mrs. J. is the support of her family and the perception by society. She experienced terrible anxiety and had to go through this ordeal on her own. Her family does not support her having any treatments done and judge her decisions which is not uncommon throughout society. So, when this incident occurred, she had very few people she could turn to for support. I offered as much as I could, but she still went home where she had to hide her injuries and felt very vulnerable and alone. I understand the majority of arguments against treatments and also support some of them. However, I truly believe that every individual is unique and understand their ageing concerns. They don’t want to fade and want to remain on the outside the way they feel on the inside. When we look good, we feel good and if there is a discord it affects our mental wellbeing. Not all aesthetic treatments are performed by cowboys. It gets a lot of bad press in the media, but it is because the bad things are what makes the news.
The majority of us are well qualified and experienced to deliver various treatments. We also deeply care for our patients and believe in the procedures and the effects it will have on an individual especially their wellbeing. I know Mrs. J’s family care for her but can confidently say so do I (or we). Let’s meet halfway by respecting our patients having these treatments without being judged and trusting that there are people who know what they are doing and will give their very best.
I am continuing to monitor Mrs. J for the next 48 hours and will remain available throughout. However, the danger has passed. A few things to take away from this. I am thankful for my training and will continue to attend and stay up to date to always feel in control for every eventuality. However, patients also need to know that what they sign up to can be risky and result in adverse events, no matter how experienced the injector is. So, if you choose to have treatments there are risks involved but you can reduce it by going to someone that are qualified and experienced. To fellow injectors – “You have got this”! You have experience and have been trained by the best. To those injectors that don’t continue learning and think that one day is enough to prepare you, you need to think again.

This is why you are so amazing You Care and respect your clients and have the skills ability knowledge and tools to deal with whatever comes your way and sometimes things do go wrong I would hazzard a guess that not all would be able to do what you have done today Can not recommend your clinic enough

UniSkin Wellness Clinic

Amazing Obagi results. Jessie is in her 20’s and was very self conscious about the pigmentation in her skin. She spent a lot of money on foundation and blending. We started her on Obagi and this is the result after 6 weeks. She still has a couple of weeks to go and not on full strength yet. However, Jessie is delighted with her results and not bothered with hiding her skin under foundation anymore. Her skin is clearer and the texture and tone much improved. An added bonus is that it also reduced her lines and wrinkles. Look at the lines on her forehead and how her eyes have opened. This has been achieved on skin treatment alone. ... See MoreSee Less

Chatting to my clients about why they seek treatments and what it means for them. The majority of you are doing it for yourself because it makes you feel better. You feel rejuvenated. Covid restrictions on aesthetic clinics have been lifted. We are allowed to open our doors again from next week. The Covid risk assessments and policies are all in place and the trial run we had seemed to be working well and effective. I have done everything to minimise the risks and help my clients remain safe. We are taking bookings again. For those clients that I had to reschedule last week, you are my main priority and we will be in touch from Tuesday to reschedule you as soon as possible. So in between opening and shutting clinic due to so many mixed messages the penultimate goal remains.
